Primary Care First
Crozer Health is participating in a new program to enhance your care.
Most Crozer Health primary care practices that were enrolled in Comprehensive Primary Care Plus (CPC+) have transitioned to a new model called Primary Care First (PCF) as of January 1, 2022.
The Centers for Medicare and Medicaid Services (CMS) offers their new PCF program, which builds on CPC+ to help us further transform our practices to meet the changing needs of the patients we serve.
Through PCF, we will have the freedom to innovate our care delivery approach based on our patients and resources. PCF provides additional support to ensure we have the infrastructure needed to enhance and personalize our work, delivering the highest quality patient-centered care. Enrolled Crozer Health practices have greater financial resources and flexibility to invest in improving the quality and efficiency of care while reducing unnecessary health care utilization.
